Immunological checkpoint inhibitors have previously demonstrated  their effectiveness and safety in various solid tumors, however, with regard to stomach cancer, to date, ambiguous results have been presented. Tumor cells express programmed death ligand 1 (PD-L1), which binds to its programmed death receptor 1 (PD-1). Immune defense plays a key role in the initiation and progression of the disease. Understanding the regulatory mechanism of PD-L1 in gastric cancer can lead to significant progress in immunotherapy, as well as contribute to the adequate selection of patients treated with checkpoint inhibitors. In the review, we conducted an in-depth study of PD-L1 expression and regulatory immunosuppressive mechanisms in gastric cancer and methods for assessing PD-L1 status, and also studied the results of current clinical trials in which inhibitors of immunological control points were considered in combination with and without chemotherapy for this oncopathology.</p></abstract><trans-abstract xml:lang="ru"><p>Рак желудка в настоящее время является одним из самых распространенных онкологических заболеваний, что обусловливает значимость и актуальность исследований в этой области. ингибиторы иммунологических контрольных точек ранее продемонстрировали свою эффективность и безопасность при различных солидных опухолях, однако, что касается рака желудка, на сегодняшний день представлены неоднозначные результаты. клетки опухоли экспрессируют лиганд запрограммированной  смерти 1 (PD-L1), который связывается с рецептором запрограммированной смерти 1 (PD-1). иммунная защита играет ключевую роль в инициации и прогрессировании заболевания. понимание регуляторного механизма PD-L1 при раке желудка может привести к существенному прогрессу в иммунотерапии, а также способствовать адекватному отбору пациентов для лечения ингибиторами иммунологических контрольных точек. в обзоре мы провели углубленное исследование экспрессии PD-L1 и регуляторных иммуносупрессивных механизмов при раке желудка, методов оценки PD-L1-статуса, а также изучили результаты текущих клинических испытаний, в которых были рассмотрены ингибиторы иммунологических контрольных точек в комбинации с химиотерапией и без нее при данной онкологической патологии.</p></trans-abstract><kwd-group xml:lang="en"><kwd>gastric cancer</kwd><kwd>immune cells</kwd><kwd>ligand of programmed cell death</kwd><kwd>tumor</kwd><kwd>programmed death ligand 1</kwd><kwd>receptors</kwd><kwd>signaling pathways</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>рак желудка</kwd><kwd>иммунные клетки</kwd><kwd>лиганд программируемой клеточной гибели 1</kwd><kwd>опухоль</kwd><kwd>рецепторы</kwd><kwd>сигнальные пути</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Bray F., Ferlay J., Soerjomataram I. et al.
