Tissue for genetic analysis may be obtained either by surgical excision or through fine-needle aspiration biopsy (FNAB). Performing genetic testing and FNAB in each institution can differ in surgical techniques and laboratory methodologies.</p><p><bold>Aim.</bold> To present our own experience of performing FNAB-based molecular genetic testing for prognostication in patients with uveal melanoma.</p><p><bold>Materials and methods.</bold> Prognostic FNAB (n = 151) were combined with brachytherapy or stereotactic surgery. Genetic testing was performed by methods based on polymerase chain reaction (<italic>GNAQ, GNA11, EIF1AX</italic> and <italic>SF3B1</italic> mutations) and fluorescence <italic>in situ</italic> hybridization (copy numbers of <italic>PPARG</italic> and <italic>MYC</italic> genes); cytology of <italic>FNAB</italic> material was also assessed.</p><p><bold>Results.</bold> Fine-needle aspiration biopsy material was informative in 91 % of cases. At the median follow-up of 36 months, 12 cases of distant metastases were detected. Occurrence of the assessed mutations and copy numbers were related to other representative studies. <italic>PPARG</italic> deletion was shown to be a significant prognostic factor for metastasis-free survival (p &lt;0,01), which was demonstrated for the first time; <italic>EIF1AX</italic> and <italic>SF3B1</italic> mutations, <italic>MYC</italic> amplification and cytological class were not shown to be significantly associated with survival in our study.</p><p><bold>Conclusion.</bold> FNAB-based molecular genetic testing for prognostication in patients with uveal melanoma was shown to be a reliable and highly informative approach. Some of the prognostic factors need to be evaluated further with longer follow-up.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> Для определения прогноза заболевания при увеальной меланоме активно используются молекулярно-генетические методы исследования материала первичной опухоли, который может быть получен в рамках органосохраняющего лечения с помощью тонкоигольной аспирационной биопсии (ТИАБ). Молекулярно-генетическое тестирование и ТИАБ в отечественной практике имеют ряд особенностей, связанных с хирургической техникой, методикой молекулярно-генетического тестирования и классификационными подходами.</p><p><bold>Цель </bold>– представить собственный опыт применения молекулярно-генетических методов в прогнозировании риска развития метастазов у пациентов с увеальной меланомой на основе материала ТИАБ.</p><p><bold>Материалы и методы.</bold> Был проанализирован материал ТИАБ, полученный от 151 пациента. Тонкоигольную аспирационную биопсию с прогностической целью выполняли одновременно с органосохраняющим лечением (брахитерапией или стереотаксической радиохирургией). В ходе молекулярно-генетического исследования осуществляли анализ мутаций в генах <italic>GNAQ, GNA11, EIF1AX</italic> и <italic>SF3B1</italic> (методами высокочувствительной мутационно-специфической полимеразной цепной реакции в реальном времени и полимеразной цепной реакции с последующим секвенированием продуктов этой реакции по Сэнгеру) и количества копий генов <italic>PPARG</italic> и <italic>MYC</italic> (методом флуоресцентной гибридизации <italic>in situ</italic>). Также проводили цитологическое исследование всех образцов, полученных при ТИАБ.</p><p><bold>Результаты.</bold> Уровень информативности материала ТИАБ составил 91 %, медиана срока наблюдения – 36 мес. За этот период отдаленные метастазы выявлены у 12 пациентов из 151. Частота встречаемости молекулярно-генетических нарушений в целом соотносится с данными других крупных исследований. Оригинальная прогностическая панель впервые продемонстрировала возможность определения прогноза пациента в зависимости от делеции гена <italic>PPARG</italic>, наличие которой ассоциировано с низкими показателями выживаемости (p &lt;0,01). Мутации в генах <italic>EIF1AX</italic> и <italic>SF3B1</italic>, амплификация гена <italic>MYC</italic> и клеточный тип опухоли по результатам цитологического исследования не продемонстрировали статистической значимости.</p><p><bold>Заключение.</bold> Использованные нами у пациентов с увеальной меланомой технологии ТИАБ и молекулярно-генетического тестирования продемонстрировали надежность и высокую информативность.
