However, despite the valuableness of molecular mRNA profiling for understanding biological behavior of the tumor, its implementation in routine clinical practice is complicated due to technological complexity and high cost of genomic sequencing. Therefore, determination of BC molecular subtype based on immunohistochemical examination can be considered an alternative to mRNA profiling. However, the method should be validated using clinical material.<bold>Aim.</bold> To evaluate prognostic significance of immunohistochemical method in determination of urothelial cancer molecular subtype using a surrogate panel consisting of 13 markers and semiquantitative calculation of the histochemical index.<bold>Materials and methods.</bold> The retrospective cohort study included 49 patients with BC who underwent radical cystectomy (RC) after previous transurethral resection (TURBT) between 2013 and 2016 at the center. The inclusion criteria were patient age between 18 and 75 years, histologically verified BC, and availability of formalin-fixed paraffin embedded blocks after TURBT and RC at the Clinical Laboratory of Morphology. The exclusion criteria were rare histological types of BC, grade IV–V surgical complications per the Clavien–Dindo classification during hospitalization, TURBT performed at other medical facilities. Molecular subtypes were determined using the immunohistochemical method on the Ventana BenchMark XT (Roche, USA) immunostainer per the traditional technique for deparaffinized sections with subtype-specific panel consisting of 13 antibodies recommended by the Lund taxonomy (LundTax). Depending on the hyperexpression level of basal and/or luminal antibodies, 4 urothelial cancer subtypes were identified: luminal А (UroA), luminal В (UroB), basal and genomically unstable (GU). The first endpoint of the study was 5-year recurrence-free survival on TURBT and RC material, secondary endpoint was 5-year overall survival on the same material.<bold>Results.</bold> Using immunohistochemical analysis with a surrogate marker panel of preserved histological material after TURBT, urothelial cancer subtype was determined in 38 (77.6 %) patients, after RC – in 39 (79.5 %) patients. Percentages of UroA, UroB and GU subtypes after TURBT and RC were almost identical; the rarest type was Basal with 4 (8.2 %) and 5 (10.2 %) cases, respectively. Evaluation of the primary endpoint showed that 5-year recurrence-free survival after TURBT (<italic>log-rank</italic> test; <italic>p</italic> = 0.85) and RC (<italic>log-rank</italic> test; <italic>p</italic> = 0.95) did not differ in various urothelial cancer subtypes. Evaluation of the secondary endpoint did not show significant statistical difference in 5-year OS<sub>1</sub> (<italic>log-rank</italic> test; <italic>p</italic> = 0.94) and OS<sub>2 </sub>(<italic>log-rank</italic> test; <italic>p</italic> = 0.92). Multivariate regression analysis showed that the most significant predictors of BC recurrence after radical treatment were clinical stage IIIA (<italic>p</italic> = 0.017) and pathomorphological stage II (<italic>p</italic> = 0.021), while OS rates were significantly affected by pathomorphological stages IIIA (<italic>p</italic> = 0.003) and IVA (<italic>p</italic> = 0.019).<bold>Conclusion.</bold> Determination of urothelial cancer molecular subtype using a surrogate panel of 13 markers and semiquantitative calculation of the histochemical index did not show effectiveness and prognostic significance: the identified 4 subtypes of urothelial cancer did not significantly affect long-term oncological outcomes.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> Результаты геномного профилирования мышечно-инвазивного рака мочевого пузыря (РМП) на основе выделения матричной РНК (мРНК) продемонстрировали значительное молекулярное разнообразие опухолей, объясняющее широкий спектр клинических проявлений и различные ответы на традиционные методы лечения. Однако, несмотря на ценность молекулярного профилирования мРНК для понимания биологического поведения опухоли, его внедрение в рутинную клиническую практику затруднено из-за технологической сложности и высокой стоимости методики геномного секвенирования. поэтому определение молекулярного подтипа РМП на основе иммуногистохимического исследования может рассматриваться как альтернатива мРНК-профилированию, однако необходима апробация метода на клиническом материале. <bold>Цель исследования</bold> – оценка прогностической значимости иммуногистохимического метода при определении молекулярного подтипа уротеалиального рака с использованием суррогатной панели из 13 маркеров с помощью метода полуколичественного расчета гистохимического индекса. <bold>Материалы и методы.</bold> В ретроспективное когортное исследование вошли 49 пациентов с РМП, которым с 2013 по 2016 г. после предшествующей трансуретральной резекции (ТУРМП) в условиях одного центра выполнена радикальная цистэктомия (РЦЭ). критериями включения в исследование были возраст больных от 18 до 75 лет, гистологически верифицированный Рмп и наличие в научной лаборатории морфологии опухолей на момент проведения иммуногистохимического исследования фиксированных в формалине и залитых в парафин блоков после ТУРМП и РЦЭ. критерии исключения: редкие гистологические варианты Рмп, осложнения хирургического лечения IV–V степени по классификации Clavien–Dindo в период госпитализации, проведение ранее ТУРМП в других медицинских учреждениях. Определение молекулярных подтипов выполнено с помощью иммуногистохимического метода на иммуностейнере Ventana BenchMark XT (Roche, США) по классической методике на депарафинизированных срезах с использованием подтип-специфической панели из 13 антител, рекомендованной таксономией лунда (LundTax). В зависимости от степени гиперэкспресии базальных и/или люминальных антител выделены 4 подтипа уротелиального рака: люминальный А (UroA), люминальный В (UroB), базальный (Basal) и геномно нестабильный (GU). первичной конечной точкой исследования были показатели 5-летней безрецидивной выживаемости на материале после ТУРМП и РЦЭ, вторичной конечной точкой – 5-летней общей выживаемости на этом же материале.<bold>Результаты</bold>. С помощью иммуногистохимического анализа с использованием суррогатной панели маркеров на сохраненном гистологическом материале после ТУРМП удалось определить подтип уротеалиального рака у 38 (77,6 %) пациентов, после РЦЭ – у 39 (79,5 %). Доли подтипов UroA, UroB и GU после ТУРМП и РЦЭ были практически равными; реже всего выявлялся подтип Basal – 4 (8,2 %) и 5 (10,2 %) случаев соответственно. при оценке первичной конечной точки 5-летняя безрецидивная выживаемость после ТУРМП (<italic>log-rank</italic>-тест; <italic>p</italic> = 0,85) и РЦЭ (<italic>log-rank</italic>-тест; <italic>p</italic> = 0,95) не различалась между выявленными подтипами уротелиального рака. при оценке вторичной конечной точки статистической разницы в 5-летней ОВ<sub>1</sub> (<italic>log-rank</italic>-тест; <italic>p</italic> = 0,94) и ОВ<sub>2</sub> также не установлено (<italic>log-rank</italic>-тест; <italic>p</italic> = 0,92). Результаты многофакторного регрессионного анализа продемонстрировали, что наиболее значимыми предикторами рецидива уротелиального рака после радикального лечения являются клиническая стадия IIIA (<italic>p</italic> = 0,017) и патоморфологическая стадия II (<italic>p</italic> = 0,021), а на показатели ОВ существенное влияние оказывали патоморфологические стадии IIIA (<italic>p</italic> = 0,003) и IVA (<italic>p</italic> = 0,019).<bold>Заключение.</bold> Определение молекулярного подтипа уротелиального рака с использованием суррогатной панели из 13 маркеров методом полуколичественного расчета гистохимического индекса не продемонстрировало эффективности и прогностической значимости: выделенные нами 4 подтипа уротелиального рака не оказали значимого влияния на отдаленные онкологические показатели.</p></trans-abstract><kwd-group xml:lang="en"><kwd>bladder cancer</kwd><kwd>urothelial cancer molecular subtype</kwd><kwd>bladder cancer treatment</kwd><kwd>immunohistochemical examination</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>рак мочевого пузыря</kwd><kwd>молекулярный подтип рака мочевого пузыря</kwd><kwd>лечение рака мочевого пузыря</kwd><kwd>иммуногистохимическое исследование</kwd></kwd-group><funding-group><funding-statement xml:lang="en">The protocol of the study was approved by the committee on biomedical ethics of N.N.
