Treatment of patients includes surgical resection, radiation and the alkylating agent temozolomide (TMZ). The therapeutic efficacy of TMZ is due to its ability to damage DNA and induce apoptosis, but it is neutralized by the expression of the DNA repair enzyme O<sup>6</sup>-methylguanine-DNA-methyltransferase <italic>(MGMT).</italic> Methylation of the <italic>MGMT</italic> gene promoter suppresses the synthesis of the corresponding enzyme and increases the cytotoxic efficiency of TMZ.<bold>Aim.</bold> To determine the <italic>MGMT</italic> promoter methylation in glioblastoma patients and to evaluate the prognostic significance of this phenomenon.<bold>Materials and methods. </bold>Bisulfite-treated DNA samples isolated from formalin-fixed paraffin-embedded tumor tissues obtained from glioblastoma patients were analyzed. <italic>MGMT</italic> methylation was assessed by qualitative methylation-specific polymerase chain reaction. The prognostic significance of this phenomenon in conjunction with a number of other clinical parameters was assessed by means of univariate and multivariate analysis.<bold>Results.</bold> <italic>MGMT</italic> promoter methylation was found to be one of the most significant favorable prognostic factors of glioblastoma: the likelihood of disease reappearance or a fatal outcome at a specific point in time is approximately two-fold lower in such patients than in those with intact <italic>MGMT.</italic><bold>Conclusion.</bold> The methylation-specific polymerase chain reaction, which is routinely used in clinical practice, adequately assesses <italic>MGMT</italic> methylation status as a prognostic factor, but it does not allow for the evaluation of its predictive potential.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> Глиобластома является наиболее распространенной злокачественной опухолью головного мозга у взрослых и характеризуется неблагоприятным прогнозом. Лечение пациентов с данной патологией включает хирургическую резекцию, облучение и применение алкилирующего агента темозоломида (TMZ). Терапевтическая эффективность последнего, обусловленная способностью повреждать ДНК и индуцировать апоптоз, нейтрализуется экспрессией фермента репарации ДНК O<sup>6</sup>-метилгуанин-ДНК-метилтрансферазы <italic>(MGMT).</italic> Метилирование промотора гена <italic>MGMT</italic> подавляет синтез этого фермента и повышает цитотоксическую эффективность TMZ.<bold>Цель исследования</bold> – выявление феномена метилирования промотора <italic>MGMT</italic> у пациентов с глиобластомой и оценка его прогностической значимости.<bold>Материалы и методы. </bold>Проанализированы обработанные бисульфитом образцы ДНК, выделенные из заключенной в парафиновые блоки опухолевой ткани. Метилирование <italic>MGMT</italic> выявляли с помощью качественного метода метилспецифичной полимеразной цепной реакции. Прогностическую значимость этого феномена в совокупности с рядом других клинических показателей оценивали с использованием однофакторного и многофакторного анализов.<bold>Результаты.</bold> Установлено, что метилирование промотора <italic>MGMT</italic> является одним из наиболее значимых благоприятных прогностических факторов глиобластомы: риск развития рецидива заболевания или летального исхода в определенный период времени у таких больных примерно в 2 раза ниже, чем у пациентов с интактным <italic>MGMT.</italic><bold>Заключение.</bold> Метил-специфичная полимеразная цепная реакция, рутинно применяемая в клинической практике, дает возможность оценить статус метилирования <italic>MGMT</italic> как фактор прогноза, но не позволяет судить о его предсказательном потенциале.</p></trans-abstract><kwd-group xml:lang="en"><kwd>glioblastoma</kwd><kwd>methylation-specific polymerase chain reaction</kwd><kwd><italic>MGMT</italic> methylation</kwd><kwd>temozolomide</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>глиобластома</kwd><kwd>метил-специфичная полимеразная цепная реакция</kwd><kwd>метилирование <italic>MGMT</italic></kwd><kwd>темозоломид</kwd></kwd-group><funding-group><funding-statement xml:lang="en">It is funded under the state budget theme.</funding-statement><funding-statement xml:lang="ru">Финансируется в рамках госбюджетной темы</funding-statement></funding-group></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Gibson D., Vo A.H., Lambing H. et al.
