Despite standardized therapeutic approaches, the platinum-free interval, progression-free survival, and overall survival vary substantially. Beyond somatic tumor characteristics and the completeness of cytoreduction, a significant portion of this variability may be driven by germline single-nucleotide polymorphisms (SNPs) in genes encoding proteins responsible for drug transport, detoxification, and DNA repair. The objective of this review is to systematize data on the molecular mechanisms of platinum sensitivity and resistance in serous ovarian cancer, with an emphasis on the role of germline structural DNA variants (SNPs), discuss the limitations of existing data, and outline directions for future research.</p> <p>A literature search was performed in PubMed, Scopus and Google Scholar using the following keywords: “high-grade serous ovarian cancer”, “platinum”, “carboplatin”, “cisplatin”, “polymorphism”, “SNP”, “pharmacogenetics”, “progression-free survival”, and “platinum resistance”. Priority was given to studies with a sample size of ≥100 ovarian cancer patients and those with independent validation or a meta-analysis. A number of studies with smaller sample sizes were included due to the particular significance of their results and are specified in the text. The most reproducible associations with platinum resistance are demonstrated by SNPs in genes involved in nucleotide excision repair pathways (ERCC1, ERCC5), platinum transport and excretion (ABCB1), and specific signaling pathways (STAT3, FGFR4). Many candidate associations remain controversial owing to variations in the definition of platinum resistance, small sample sizes, heterogeneity in treatment regimens, and interethnic differences in allele frequencies and effects.</p>
Многие кандидатные ассоциации остаются противоречивыми из-за вариации определений платинорезистентности, малых выборок, неоднородности по режимам терапии и межэтнических различий в частоте аллелей и эффектах.</p></trans-abstract><kwd-group xml:lang="en"><kwd>serous ovarian cancer</kwd><kwd>mechanisms of platinum resistance</kwd><kwd>genetic polymorphism</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>серозный рак яичников</kwd><kwd>механизмы платиновой резистентности</kwd><kwd>генетический полиморфизм</kwd></kwd-group><funding-group><award-group><funding-source><institution-wrap><institution xml:lang="ru">Министерство науки и высшего образования Российской Федерации</institution></institution-wrap><institution-wrap><institution xml:lang="en">Ministry of Science and Higher Education of the Russian Federation</institution></institution-wrap></funding-source><award-id>075-03-2025-069</award-id></award-group><funding-statement xml:lang="en">The work was performed within the framework of the state assignment of the Institute of Chemical Biology and Fundamental Medicine of the Siberian Branch of the Russian Academy of Sciences (No. 125012300674-9).</funding-statement><funding-statement xml:lang="ru">Работа выполнена в рамках государственного задания ФГБУ «Институт химической биологии и фундаментальной медицины Сибирского отделения Российской академии наук» (№ 125012300674-9).</funding-statement></funding-group></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Ovarian cancer/fallopian tube cancer/primary peritoneal cancer.
